The Belarus visa is issued and valid according to the flight/train itinerary submitted with the visa application.

You must send your passport – a copy is not acceptable. 

• Your signed passport must have more than 6 months validity remaining to apply for a visa. Visa will not be issued with less than 6 months validity.
• Must contain sufficient blank visa pages. Visas are affixed to the blank "Visa" pages of your passport. "Amendment" pages (clearly marked as such) in the back of your passport are not acceptable for visa issuance.

Please submit one (1) recent passport photograph taken within the last six months.
Please carefully review the Passport Photo Style Regulations.

Important: Please do not cut/trim & staple the photo to the visa application. You can simply attach it with a paper clip or send it loose and we will attach it for you.

Belarus Visa Application Form

Visa to next destination. The visa must already be in your passport or submit a copy of your ticket indicating the next destination or other evidence of onward travel within 48 hours of entry to Belarus.

Starting July 1st, 2014, Belarus Embassy requires every applicant to provide proof of medical insurance policy for the duration of your visit. This document should contain:

• Name, address & phone number of your insurance company, as well as your full name printed as policy holder
• Insurance policy number
• Insurance territorial coverage should be valid in Belarus or worldwide
• Insurance term of validity (should be valid through the period of the travel or temporary residence in Belarus
• Amount of medical coverage should not be less than 10 000 Euro.

You must provide a copy of ONE of the following items:
• A computer generated flight confirmation from your airline or a travel agency indicating the dates of entry and exit from the country you are visiting. Your name must be on the itinerary OR;
• A copy of your airline ticket indicating the dates of entry and exit from the country you are visiting. Your name must be on the ticket (do not send original ticket)

If you formerly had USSR or Belarus citizenship, please submit the documents proving the loss of USSR/Belarusian citizenship.
