IMPORTANT: Your passport must have more than 6 months validity remaining after your travel date in order to apply for a visa. It must also contain at least one blank visa page for entry & exit stamps.
Please scan a clear and legible color copy of the information pages of your passport: Passport Scan Example. This must be uploaded, or emailed to us, during the application process. Please do not take a picture of your passport info page- it must be scanned instead.
If your passport has been amended (name changed, validity extended—found in the back pages of your passport) you will also need to scan and upload along with your passport information pages.
A passport photo will need to be uploaded, or emailed to us, as a part of the application process. You may either:
(1) Scan a recent 2x2 passport photograph that meets Passport Photo Style Regulations. Please don't take a picture of your passport photo—the passport photo must be scanned; OR
(2) Take a digital photo using a camera. You must attach the original file size (100KB or larger)- thumbnail size is not accepted. Please make sure the picture is taken against a light/white background—Sample Photo Click Here
Proof of sufficient funds (recent bank statement). Please make sure you white out or otherwise remove or black out the account number from the statement.

From host, sponsor or company to be visited in Cameroon stating responsibility for and acknowledgement of your stay in Cameroon.
The letter must:

State the specific visa type & validity being requested. Long stay visa applicant: if you are applying for a 6-month or 1-year multiple-entry visa, the exact duration of stay and type of visa should be explicitly mentioned on the invitation.

Be legalized* by thoroughly identified business partner(s) in Cameroon, undersigned by Immigration or Police Authorities.
*For a letter to be "legalized" it must be "notarized" by a local authority in Cameroon. The seal of the notary must be on the letter: Cameroon Legalized Letter (Sample).
The following may also be requested:
Business contract between traveler(s) and partner(s) in Cameroon, undersigned by Administrative or Immigration Authorities.

If staying with a relative or friend, please submit a letter of explanation from host along with host's passport/national ID.

Applicants under age 18 must submit the following additional documents, even if both parents are traveling with minor:

(1) If you've received a visa: submit a scan copy of most recent Cameroonian visa.
(2) Naturalized U.S. citizens born in Cameroon who are first time visa applicants must surrender their former Cameroonian passport to the Embassy after we've submitted your request.
Send it to Embassy of Cameroon (FedEx/UPS/USPS with tracking), Attn: Consular Section E-Visa, 2349 Massachusetts Ave NW, Washington, DC 20008 and include a note bearing your name, phone number, visa application number, and indicate that the Cameroonian passport is sent as part of an E-visa application.
Note that due to this, visa processing time will take a bit longer.
(3) If your Cameroonian passport is lost, submit a scan copy of the police report showing case number, indicating that it was lost.
(4) If the Cameroonian passport was retained by U.S. Authorities (Immigration or other), provide a scan copy of attestation from said Authorities confirming that fact.
U.S. citizens born in Cameroon or of Cameroonian origin applying for a long stay visa (6-month or 1-year) must comply with the requirements set out above for visitors requesting a long stay visa.
