DS-5504 Quick Start Application Instructions:

After the DS-5504 Passport Amendment Application Form opens:
  • Check the box next to "I have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaimers" and click Submit.
  • Choose to "Apply Online" and click Submit
  • Complete the required fields and click "create form".
  • Print the resulting document twice.
  • Close the document.
  • Do not forget to sign the documents once they have printed.
